I hereby voluntarily register as a prospective blood donor with The Voice Of Sikkim's Bipashana donor registry. I confirm I am between 18 and 65 years of age, in general good health, and that this registration is entirely voluntary and non-remunerated, in line with the National Blood Transfusion Council (NBTC) and NACO guidelines on voluntary blood donation. I understand that submitting this form does not by itself confirm my eligibility to donate -- final eligibility will always be decided by a qualified medical officer through mandatory pre-donation screening (haemoglobin, blood pressure, weight and health history) at the time of an actual donation. I consent to my name, contact number and blood group being shared only with hospitals, blood banks and authorised coordination volunteers, and only for the purpose of contacting me during a genuine emergency blood requirement within Sikkim. I understand I may withdraw this consent and request deletion of my details at any time by contacting The Voice Of Sikkim. I confirm that the details I have provided above are true and accurate.
